recognized, locally implemented
program of the City of Fremont
Human Services, funded by a two
year grant (2007-09) from the
Robert Wood Johnson Foundation.

 The
Community Ambassador Program for
Seniors, through its volunteer
community ambassadors, will
build capacity to serve seniors
in their own communities, in
their own language, within their
own cultural norms, and will do
so where seniors live, worship,
socialize, and learn. The
Ambassadors serve as a bridge
between the formal network of
social services and faith based
and cultural communities. Visit

As of July 2010, 138 ambassadors have been trained.
Pathways to Positive Aging
The CAPS program is part of the
larger Robert Wood Johnson
strategic grant funded project,
Pathways to Positive Aging, a
project of the City of Fremont
Human Services and Tri-City
Elder Coalition, whose goal is
to empower seniors to
understand, choose, and access
culturally enriched, affordable
services and opportunities that
enhance their quality of life.
